China’s Sexual and Reproductive Health and Family Planning in Reform -- A Review of ICPD+25

Universal access to sexual and reproductive health and family planning is one of the key objectives set at ICPD in Cairo, 1994.

Over the last 25 years, the Chinese Government has made unremitting efforts towards implementation of ICPD Program of Action and to ensure people-centered and quality reproductive health and family planning services. It has made impressive progress in improving people's health and wellbeing.

In 1995, the Chinese Government initiated a reform of its national family planning program for the purpose of realizing the fundamental change from an emphasis on demographic targets towards client-centered approaches. With the support of international organizations including UNFPA and Ford Foundation, the Government launched a pilot Project of Quality of Care in Family Planning first in six counties and then gradually scaled it up to the whole country.

With the adoption of the Population and Family Planning Law in 2001, the Chinese Government shifted its focus from controlling the size of the population to promoting all-round development of the people.

The capacity building of service providers has always been a persistent focus of China's program . A nationwide initiative has been made to develop a group of licensed consultants who are willing to serve the people, capable in good communication and skilled in service provision. These licensed consultants have made great contribution to assisting couples at reproductive age to choose appropriate contraceptive methods on the basis of informed choice.

The gender perspective has been introduced into the service provision process, with particular focus on male participation and couple's joint decision on family planning and family development.

One of China’s main approaches in ensuring the quality of care is to build the national norms for standardized and client-centered services. The State has introduced and implemented 33 indicators for supervising and evaluating the quality of care.

In remote rural areas, efforts have been made to improve knowledge on reproductive health, maternal and child health in a way that villagers can easily accept and understand. Hospital delivery is strongly promoted.

Adolescent sexual and reproductive health constitutes an important component of China's program. A two-decade long nationwide Project on Adolescent Sexual and Reproductive Health Education carried out by China Family Planning Association has benefited millions of teenagers since its launching in 1998.

China now has the largest size of domestic migrant population in the world. Since 2006, a nationwide program has been launched to promote social integration of migrants and provide them with equal access to basic public health services, including free contraceptives and health counseling services.

With the introducing the conditional two-child fertility policy in 2013 and the universal two-child policy in 2016, and restructuring of government departments, has brought new opportunities and requirements. The Government has integrated reproductive health and family planning services into the public health service system, and doubled its efforts in improving the social supporting system for family development and healthy ageing. A series of special reproductive health and contraceptive services have been provided in response to the adjustment of fertility policy.

To address the emerging challenge of rapid increase of high-risk pregnancy associated with the fertility policy adjustment since 2016, China has initiated a national scheme called the "Five Regulations for Maternal and Infant Safety", which focuses on preventing and reducing maternal and infant mortality.

There are a number of emerging challenges facing China to provide quality reproductive health and family planning services in the context of low fertility.

Looking forward, the Chinese Government will make continuous efforts for the realization of the ICPD Program of Action beyond 2014 and the 2030 Sustainable Development Goals. China is dedicated to building a Healthy China and advancing the international cooperation, especially South-South cooperation, so as to make new contributions to the health and well-being of people around the world.
